EMPLOYEE BENEFITS SUMMARY - UNITED STATES
At FocusFrame, we offer a full range of benefits designed to meet our employee’s needs. The following is a summary of benefits offered to full-time employees in the United States.
MEDICAL COVERAGE
FocusFrame offers the opportunity to participate in a Preferred Provider Organization (PPO) plan through BlueCross. Employees are eligible for medical coverage the first of the month after their date of hire.
DENTAL COVERAGE
FocusFrame offers the opportunity to participate in a group dental plan through Humana Dental. Employees are eligible for dental coverage the first of the month after their date of hire.
LIFE INSURANCE
Effective the first of the month following their date of hire, employees are covered under a group life insurance plan through Reliance Standard Life Insurance Company. This benefit is provided by FocusFrame at no cost to the employee.
SHORT TERM DISABILITY (STD)
Effective the first of the month following their date of hire, employees are covered under STD insurance. This benefit is provided by FocusFrame at no cost to the employee.
LONG TERM DISABILITY (LTD)
FocusFrame also has LTD coverage for employees who exceed 90 consecutive days of total disability. This benefit is provided by FocusFrame at no cost to the employee.
VISION PLAN
In connection with our medical coverage, employees can receive up to 65% off of frames, lenses and exams through the Blue Cross Healthy Extensions network.
401(k)
Beginning the first of the month, after six months of continuous employment, employees are eligible to participate in FocusFrame’s 401(k) Plan. This is a self-managed plan through ADP and the company provides a 50% match on employee contributions up to the first six percent of eligible wages.
VACATION
Effective their date of hire, employees begin accruing vacation time. Vacation time is accrued twice a month in connection with our scheduled paydays. The maximum vacation accrual that can be carried is 200 hours.
SICK TIME
Effective January 1, employees are granted 5 days (40 hours) of sick time. Employees hired during the calendar year are granted a pro-rated amount for that year, which is calculated based on their date of hire. Sick time does not carry over from one year to the next.
HOLIDAYS
FocusFrame observes up to 11 company paid holidays per calendar year. These holidays include New Years Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day After Thanksgiving and Christmas Day. In addition, employees who are onboard before July 1 are eligible for one floating holiday in that calendar year.
SECTION 125 PLAN
Employees are eligible to participate in our Section 125 Plan. This plan allows the employee to put aside money, before taxes, to pay for medical and dental expenses. Employees can also utilize this plan to have any monthly medical and/or dental premiums deducted pre-tax rather than post-tax.
HEALTH CLUB MEMBERSHIP REIMBURSEMENT
Employees are eligible for a reimbursement of up to $100, once every twelve months, for the purchase of a health club membership. This reimbursement is only applicable for club memberships.
